5.3: Transition Frameworks & Barriers

The transition from pediatric to adult services is a complex process.

Often, doctors do not prepare their patients well for transition, and patients are resistant to letting go of their pediatric providers.

In this section, I will share two model frameworks for healthcare transition: Got Transition's “Six Core Elements of Health Care Transition” and The Child Neurology Foundation's Transition of Care program, which includes its 8 Common Principles of Transition of Care. These frameworks can help make the process more successful.

I will also share some of the barriers that may impact the success of the transition to adult healthcare in childhood-onset neurological disorders.
